HOW IT WORKS
✔️ Entry Fee of $25 per player
✔️ Each team of 5 players, per age group, 14-18 years, Grades 9 thru 12
✔️ Prize $600 to the winning team in the Final
✔️ Players can donate more that the required $25 Entry Fee, * All proceeds from players will be donated to the Epilepsy Foundation

* All proceeds from Donations and Entry Fees will be donated to the Epilepsy Foundation - https://www.epicli.org/fundraising/donation/

➡️ Click Here for the Waiver Form - SOCCER FOR EPILEPSY PARENT/GUARDIAN CONSENT AND MEDICAL CONSENT FORM.

➡️ Print the waiver form, and bring the completed and signed waiver form along with you on the day of the tournament. Thank you.
